Healthy & Active Before 5 (HAB45) is a Contra Costa County collaborative that advances health equity through local policy and environmental changes to support the health and well-being of children ages 0-5 and their families. We envision a Contra Costa where all children are free to play, learn, and grow in strong, healthy, equitable communities.

HAB45’s core assets are its strong collaborative leadership and partnerships. Early childhood health and health equity are tremendously complex public health problem that requires strategic, collaborative efforts and a host of coordinated strategies to treat and prevent. HAB45 is at the center of quality collaboration in the county, by coordinating efforts to address the social determinants of early childhood health and build health equity.
